Eastlake Community Counseling uses a secure, user friendly,24-hour online scheduling system for your greatest convenience.  You can log on at any time, day or night, to schedule your own appointment.  In our scheduling system, you have the ability to schedule a New Patient appointment or, if you have been seen before, a Follow Up visit.  You may schedule a single appointment, or up to 6 recurring appointments if you wish (this is highly recommended if your schedule is not flexible). Appointments may be made online up to 24 hours prior to the appointment time.  If you need an appointment on shorter notice or would prefer to schedule over the phone, please call and speak with Jeff directly.  In addition, appointments may be canceled online up until 48 hours prior to your appointment time.  If you need to cancel with less than 48 hours notice, please call (and keep in mind that clients canceling with less than 24 hours notice will be assessed a $50 fee).  When entering your information in the scheduling system, please put "NA" in all fields that do not apply to you and put the word "cash" in the insurance field if you plan to pay out-of-pocket.  If you are planning on using your insurance, you must enter all of the necessary insurance information.  All information entered into our appointment system is held in the strictest confidence and will not be used for any purpose other than scheduling and billing.  If you are a new client: please note that you must either download and complete your new client paperwork prior to your appointment and bring it with you, or arrive 20-30 minutes early to your first appointment to complete this paperwork.  Even if you have previously seen Jeff Palitz, MFT at PsyCare, you will still be required to complete this paperwork.  To download the necessary forms now, click here.
